central-affiliated cities.
Recently, the Ministry of Health (Standing Body of
the National Steering Committee for COVID-19 Prevention and Control) issued
Decision No. 2194/QD-BCDQG dated May 27, 2020 on guidelines for the prevention
and control of COVID-19 in the workplace and dormitories for workers and
Decision No. 2787/QD-BYT dated June 5, 2021 on guidance on plans for response
to COVID-19 cases at production and business facilities (hereinafter referred
to as businesses) and industrial parks. However,
through the implementation process, inspection and supervision in provinces and
cities, it has been shown that certain businesses have not yet developed a plan
for prevention and control of COVID-19 and response to suspected or confirmed
cases of COVID-19 at work (hereinafter referred to as COVID-19 response plan)
in accordance with regulations and not in line with the actual circumstances of
businesses. Many provinces and cities have not yet implemented regulations on
COVID-19 prevention and control in boarding houses for workers.
To unify and enable businesses and provinces and
cities to take initiative in developing the COVID-19 response plan to suspected
or confirmed cases of COVID-19 (hereinafter referred to as plan form), the
Ministry of Health issues a plan form for businesses (Appendix 1) and form of
commitment on COVID-19 control for boarding houses for workers (hereinafter
referred to as commitment form) (Appendix 2) for the People’s Committee of
province to provide specific guidelines for businesses and relevant entities.
The Ministry of Health requests People's Committees
of provinces to communicate and guide the plan form for businesses in the
provinces under their management and the commitment form for boarding houses
for workers to local governments to implement Covid control in accordance with
the actual situation and local regulations.

1. Preparedness

isolation room (specify the location, equipment). Review and separate the
private path to the temporary isolation room.
- Temperature
screening at the entrance, QR code scanning points.
- Review and add
handwashing points with soap/use rapid hand sanitizer at high-risk locations.
- Install surveillance
cameras to perform and support tracing at working locations, canteens, public

3.1. Business visitors:
- Make health
declaration according to current regulations. The division that receives the
health declaration form shall check the locations where the declarant has
traveled for the last 14 days, if the declarant has passed any place specified
in the list which is daily updated by the Covid steering board of the entity from
the website of Center for Disease Control of province/city and the Ministry of
Health), it should promptly report it to the Covid steering board of the
entity.
- Check
temperature, make facemasks and hand sanitizing mandatory
- The entity keep
records (contact persons, division, time, contact location).
Notes: If
a business visitor had been passed through a pandemic-hit area, he/she must
complete his/her mandatory quarantine period in advance. If he/she fails to complete the mandatory quarantine period, take
him/her to the temporary isolation room. Report
to the on-site workplace coordinator; contact the local health center for
further actions.

high-risk mass gathering event within 14 days (daily updated by the Covid
steering board of the entity from the website of Center for Disease Control of

+ If a worker is a F1 or F2 individual (in the
locality of residence), report to the on-site workplace coordinator to
cooperate in localizing and tracing in the workplace.
+ The health condition of isolated workers should
be reported to the on-site workplace coordinator.
+ If a worker develops suspected symptoms of fever,
cough, difficulty breathing, etc., he/she is not allowed to come to the
workplace, must report to the manager and go to the doctor. Only go to work
when symptoms are gone.

vending machines, elevator, stairs, etc.
- In public areas,
safety measures must be taken to minimize the risk of infection such as:
+ Place adequate hand sanitizers at convenient
locations and adequate soap in handwashing faucets.
+ Clean and disinfect at least 02 times/day for
frequently touched surfaces such as doorknobs, stair handrails, handrails,
elevator control panels, electrical switches, computer keyboards , shared
phones, buttons at ATMs, vending machines, public drinking water dispensers,
etc.

II. Workers
suspected or confirmed with COVID-19 at the entity
1. Any
worker suspected or confirmed with COVID-19 at the entity reported.
- Immediately
report to the director of the entity to make a decision to lock down the entire
entity temporarily or each workshop/production line/production team/production
area/work location with the COVID-19 case (F0 individual) depending on
circumstances to serve the tracing, isolation, taking samples for testing.
- Simultaneously
ask the F0 individual not to move, not to contact people around, wear a mask
and wait for someone to guide.
- Inform the
assigned task force (please specify the assigned team/individual) to separate
the path from the area with F0 individual to the temporary isolation room.
- Instruct the F0
individual to follow the separate path to the temporary isolation room.
- Inform the local
health authority to transfer the F0 individual to the health facility for
isolation and treatment
- Inform the
assigned team (please specify the assigned team/individual) to zone and

all close contacts (F1 individuals), close contacts with F1 individuals (F2
individuals) through work shift arrangements, surveillance cameras, information
from managers, COVID-19 safety teams.
- Categorize and separate
areas for F1 individuals (separate cases within the same working position and
exposure risk in group of F1 individuals), F2 individuals to prepare for
testing and isolation as per regulations.
3. Staff
member in charge…(specify full name and position…) make a list of workers as F1 individuals, F2
individuals, other cases not present at the entity at the time of lockdown to:
- Send it to the
Department of Health/CDC/health center of the district where the worker is
residing for actions according to regulations.

shutdown of the workshop from 48-72 hours to clean and disinfect, check all F1,

6.3. Scenario 3: Detect many F0 cases working in
most workshops/production lines/production teams/production areas/working
locations at the entity.
- Conduct a
temporary lockdown of the entire entity.
- Isolate F0 cases
on the spot and immediately notify the health authority for isolation,
treatment, and zoning and disinfection according to regulations.
- Urgently trace
all F1, F2 cases of new F0 cases for further quarantine according to
regulations. All workers in the entity are considered F1 individuals and sent
to centralized quarantine facilities immediately.
- Inform the

arriving at work
- Prepare personal

for Party B while ensuring COVID-19 safety protocols as follows:
I. Requirements for shuttle buses:
1. Keep the
shuttle buses well ventilated at all times. Clean
and disinfect shuttle buses at the end of each day.
2. Place hand sanitizer
(licensed by the Ministry of Health); stick QR code of the quarantine point on
the door of the bus, there are trash cans and surveillance cameras on the bus.
3. Make a list or
manage by cards, fix the seats for each person on the bus.
4. Do not carry
more than the number of people according to the issued permit, open windows and
ventilation doors if possible while moving.
5. Register
specific schedules.
6. In case of
community transmission where the business is located:
assign the rider’s name to a fixed seat to maintain the mandatory
distance according to the list.

In order to prevent and control the COVID-19
pandemic, I hereby commit to the landlord and local authority to perform well
the following:
1. Strictly make
health declarations, install Bluezone, follow 5K rules and regulations and
rules in COVID-19 prevention and control at the boarding house.
2. Do not gather
in large numbers of people in the boarding house.
3. Do not go out
of the boarding house unnecessarily.
4. Notify the
health condition when having one of the symptoms such as fever, cough, sore
throat, difficulty breathing and other health problems to the commune health
station, hotlines of the health sector and the business and industrial park.
5. Coordinate with
the Covid community team to take measures to prevent and control the COVID-19
pandemic.
I hereby commit to comply with the above
particulars strictly, in case of any breach, I will take legal responsibility. The commitment is made into 02 copies, the tenant
keeps 01 copy, the landlord keeps 01 copy,./.

COMMIT TO COMPLY WITH REGULATIONS ON COVID-19
PREVENTION AND CONTROL IN THE BOARDING HOUSE AS FOLLOWS:
1. Requirements
for boarding house:
- Ensure
ventilation, room area is not less than 5m2/person.
- There are places
to wash hands with soap or hand sanitizer at common use locations in the
boarding house.
- There are
leaflets and posters about 5K rules; post up the phone numbers of the community
Covid team, hotlines of the health sector, the manager of the boarding house.
- There are en
suite toilets, if a shared toilet is used, no more than 12 people/toilet.
- There is enough
clean water for eating and living.
- There are common
cleaning solutions, water containers/pots, mops for tenants to self-clean and
disinfect the accommodation.
- There are trash
cans with lids for daily waste collection.

control.
- Provide the list
of tenants to the People's Committee of commune on the daily basis.
