Re: initiation of solutions to reduce
administrative procedures for F0 people at health stations of wards, communes
and towns

Recently, it has been recorded a sharp increase in
COVID-19 infections, most cases are asymptomatic or have mild clinical
symptoms, eligible for home isolation. This puts heavy workload on health
care workers at the health stations and causes troubles for the people.
The Department of Health requires all health
facilities, health centers, and health stations to urgently initiate digital
transformation in confirming F0 patients and confirming the completion of home
quarantine, in specific:
1. Hospitals,
clinics, health centers, health stations
Places where people with COVID-19 are identified
such as hospitals, clinics, health centers, health stations, etc., when
identifying people with COVID-19 who have no clinical symptoms or have mild
clinical symptoms. eligible for home isolation shall: instruct people
with COVID-19 to access the address: https://khaibaof0.tphcm.gov.vn on a
computer or smartphone with internet connection to “report F0 cases”. The
declaration will be recorded by the system and sent to the health station of
the ward, commune, town (commune level) where the person with COVID-19 is
quarantined.
Health facilities must strengthen communication on
all means of communication of their units (websites, bulletin boards, leaflets,
newsletters at health departments, clinical departments, etc.) to guide
how to declare to local health when having a positive SARS-CoV-2 test result
and quarantine at home via https://khaibaof0.tphcm.gov.vn.
2. Center
for Disease Control of City (HCDC): create contents and implement, guide
units to communicate with people on how to use computers or smartphones with
internet connection to “report F0 cases” when home quarantined.

Direct the commune-level People's Committees to
strengthen the commune-level epidemic prevention and control steering
committees; assign the leader of the health station to be the deputy head of
the steering committee and confirm the certificate of completion of home
quarantine (the certificate). And mobilize personnel to support the health
station to receive reports of F0 cases and issue a certificate to F0 person in
a timely manner, limiting the concentration of people at the health station to
carry out administrative procedures.
Direct relevant departments to communicate to each
person with a positive test result for SARS-CoV-2 that they need to immediately
report it to the commune health station via https://khaibaof0.tphcm .gov.vn to
be received, cared for and managed.
Train related divisions on basic knowledge about
COVID-19 disease, instructions on caring for people with COVID-19 at home to
receive reports of F0 cases on the software.
4. Commune
health station
Assign specialized personnel in charge of receiving
SMS messages notifying the list of self-reported F0 cases; regularly log in to
the COVID-19 management platform to verify and receive the list of
self-reported cases for further care and management according to the guidance
of the Ministry of Health and the Department of Health.
Assign personnel to answer the phone (this phone
number must be different from the phone number that receives messages of
self-reported F0 cases) to receive SMS messages announcing the negative test
results declared by F0 at the end of the quarantine period ; log in to the
COVID-19 management platform to confirm the completion of the quarantine
period, sign and issue the certificates of completion to F0 cases in electronic
form via email or in paper form; limit F0 cases or their family members from
gathering at the health station to wait for the certificates to be issued.
In case the patient could not take the rapid
antigen test themselves to end the quarantine period, the health station needs
to arrange a separate area and specifically notify the patient about the time
to take samples for testing.
Based on the number of F0 cases and the number of
personnel members at the health station, the health station proposes the
commune-level People's Committee to mobilize more personnel to support the
health station to receive information on self-reported F0 cases and issue
certificates to these F0 cases.
The Department of Health requests the units to
implement the above regulations. Difficulties that arise during the
implementation of this document should be reported to the Department of Health
for support.

TRANSFORMATION FOR CONFIRMATION OF F0 CASES
1. Self-reported F0 cases
When a person with suspected symptoms of COVID-19
takes an antigen rapid test himself/herself and has a positive result for
SARS-CoV-2, instead of going to the health station to report and get
confirmation, he/she shall take the following steps:

- Take a photo of your
identity card/citizen identity card or identity paper with photo (health
insurance certificate, driver's license, birth certificate, student card...).
- Access the address:
https://khaibaof0.tphcm.gov.vn (on a computer or smartphone with internet
connection) and select "Report of F0 cases".
- Enter the mobile phone
number you are using in the box "Enter a verified phone number", then
click the "Get OTP code" button. The system will automatically
send the OTP code to the phone number entered in the form of SMS.
- Enter the OTP code and press
the “Continue” button. The system will switch to the “F0 patient information”
screen.
- Enter full personal
information (attach photo of identity card/citizen identity card or identity
paper with photo), medical history, COVID-19 vaccination status, current symptoms,
test results (attach positive test result picture).
- Upon completion, press the
"Save" button to send the declarant's information to the health
station where you live.
- The system will send the
following message to the phone number of the declarant "The system has
received the report of Mr./Ms. ……………………. The Covid Steering Committee of
COVID-19 of ward.... will contact Mr./Ms.…. to verify information and support
Mr./Ms. at the earliest. Thank you".
Notes: The report must have all information and
you shall take responsibility for the declared information.
2. The commune health
station verifies and receives home-quarantined F0 cases

- Check personal information,
medical history, vaccination history, test results, current symptoms and severe
symptoms (via F0's declaration).
- Call the F0 patient to verify
his/her name, quarantine address and home quarantine conditions.
- Click the
"receive" button after completing the above steps.
Depending on the number of home-quarantined F0
cases in the commune, the above steps can be performed by staff of the health
station, mobile health station; Covid care teams in the community, mobile
health teams, community COVID teams..... The people in charge must be trained
by the health center and/or health station.

1. Self-reported F0 cases
When the quarantine period ends, the F0 patient
takes a rapid antigen test. If the antigen test result is negative, F0
patient performs the following steps:

- Access the address:
https://khaibaof0.tphcm.gov.vn (on a computer or smartphone with internet
connection) and select “Declare completion of quarantine”.
- Enter the mobile phone
number you are using in the box "Enter a verified phone number", then
click the "Get OTP code" button. The system will automatically
send the OTP code to the phone number entered in the form of SMS.
- Enter the OTP code in the
"OTP verification code" box, then press the "Continue"
button.
- In case the F0 person’s information is already
available on the COVID-19 management digital Platform (self-reported, or
entered by health facilities on the software).
+ The system will switch to the screen "test
result after quarantine".
+ Enter test date, test result (attach negative
test result image).
+ Upon completion, press the "Save"
button to send the negative test result information during the quarantine
period to the health station where you live.
+ The system will send the following message to the
phone number of the declarant "The system has received the negative test
result of Mr./Ms. ……………………. The Covid Steering Committee of ward....
will send a certificate of home-quarantine completion via the email address
that you have declared, if you do not have an email, please go to the health
station to receive it. Thank you".
- In case the F0 person’s information is not
available on the COVID-19 management digital Platform (not yet self-reported)
at https://khaibaof0.tphcm.gov.vn when the test result is positive) [1].

+ The patient in quarantine will declare personal
information (attach photo of identity card/citizen identity card or identity
paper with photo), time of positive test result, time of negative test result
(attach a picture of negative test result).
+ Upon completion, press the "Save"
button to send the negative test result information during the quarantine
period to the health station where you live.
+ The system will send the following message to the
phone number of the declarant "The system has received the negative test
result of Mr./Ms. ……………………. The Covid Steering
Committee of ward.... will verify information and contact Mr./Ms.…. at the
earliest. Thank you".
2. The commune health
station verifies and issues certificates
On receiving a message informing F0's negative test
result, the health station staff in charge of issuing certificates shall log in
to the clinic account at https://covid19.medinet.org .vn:75 to perform the
following steps:
- Check the information that
the negative test result after the isolation period is transferred to the
COVID-19 Management Platform via the Health Station's account.
a. For the "reported F0 case" via
the address: https://khaibaof0.tphcm.gov.vn when the test result is
positive
- Select F0 name from
“Certification waiting list”.
- Click the "Finish
quarantine" button. The system will send a message "Mr./Ms. has
completed quarantine at home from ………… to …………. ”.

+ Click “Export certificate” then select the file
format you want to download, adjust the information (if necessary) and print
the certificate.
+ Leader of the health station (assigned by the
commune-level steering committee for pandemic prevention and control) signs the
certificate and affix the stamp of the health station.
+ Log in to the COVID-19 digital management
platform, select the F0 person to send an email, enter the email content and
attach a scanned copy of the signed certificate and press the "send
email" button. The electronic copy will be sent to the email of the
reported F0.
- In case the health station
digitally signs the certificate, click "digitally sign and send
email". The mail will be automatically sent to F0's email.
b. For the F0 case that “has not been
reported” via the address: https://khaibaof0.tphcm.gov.vn when the test
result is positive
- Based on the list of F0
cases in the "negative declaration list", call to verify information
or check the list of home-quarantined F0s in charged by local COVID-19 home
care facilities (mobile health stations, COVID-19 patient care teams in the
community, mobile health teams...).
- If there is evidence to
determine that F0 person has quarantined at home. The health station
presses “receive” and follows the procedure in step (a).
- If there is no evidence to
determine that F0 person has quarantined at home. The health station
clicks "refuse to issue certificate".
- The system will send a notice
to the quarantined person “The Covid Steering Committee of ward…. refuses to
issue a certificate of completion of quarantine to Mr./Ms.…………..because there
is no evidence to identify Mr./Ms. has quarantined at home”.
