New forms issued for technology appraisal in investment projects
According to the Circular, the forms used for technology appraisal or consultation on technologies in investment projects, technology transfer registration, approval and licensing of restricted technology transfer, certification of eligibility for technology assessment and inspection services and implementation reporting provided in the Appendix enclosed with this Circular include:
- Forms for explanations on technology use in investment project dossiers at the stage of investment guideline approval, issuance of Investment Registration Certificates, or investment decision-making (Forms No. 1.1 and 1.2).
- Forms for carrying out procedures for technology appraisal and consultation on technologies in investment projects at the stage of investment guideline approval or issuance of Investment Registration Certificates (Forms No. 2.1, 2.2, 2.3, 2.4, 2.5 and 2.6).
- Forms for carrying out procedures for technology appraisal and consultation on technologies in investment projects at the investment decision-making stage (Forms No. 3.1, 3.2, 3.3, 3.4, 3.5 and 3.6).
- Forms for carrying out procedures for technology transfer registration (Forms No. 4.1, 4.2, 4.3, 4.4, 4.5 and 4.6).
- Forms for carrying out procedures for approval and licensing of restricted technology transfer (Forms No. 5.1, 5.2, 5.3, 5.4, 5.5, 5.6, 5.7 and 5.8).
- Forms for carrying out procedures for issuance of Certificates of eligibility for technology assessment services (Forms No. 6.1 and 6.2).
- Forms for carrying out procedures for issuance of Certificates of eligibility for technology inspection services (Forms No. 7.1 and 7.2).
- Forms of reports on implementation of technology assessment/inspection and results of issuance of Certificates of eligibility for technology assessment and inspection services (Forms No. 8.1 and 8.2).
