According to this document, an application package for extension of a work permit for a foreign worker includes:
(1) Written request for reissuance of work permit by using the form No. 11/PLI in the Appendix I to the Decree No. 152/2020/ND-CP;
(2) 02 color photo (size 4cm x 6cm, white background, straight face, bare head, no colored glasses), taken within 06 months from the date of application;
(3) Work permit remaining valid;
(4) Written approval of needs for foreign workers, except for cases where determination of the needs for foreign workers is not required;
(5) Certified true copy of passport remaining valid under laws;
(6) Health certificate or certificate of medical examination issued by the competent healthcare establishment of the foreign country or Vietnam for use within 12 months;
(7) One of documents evidencing that the foreign worker continues to work for the employer according to the issued work permit;
Those documents prescribed in point (3), (4), (6) and (7) above must include 01 original or certified true copy. For foreigners, both consular legalization and Vietnamese translation are required, except for those eligible for consular legalization requirement.
Decision No. 526/QD-LDTBXH is entering into force as of May 6, 2021.
