Ministry of Health requests People’s Committees of provinces and cities to direct involved agencies to mandatorily use COVID-19 health declaration forms as follows:
- Passengers who enter Vietnam from or through South Korea.
- Location: All border checkpoints.
- Time: From 3 p.m., February 23, 2020.
- Health declaration form: “Form No. 01: Health Declaration Form for Passengers" in the Appendix to the Decree No. 89/2018/ND-CP dated June 25, 2018.
- Korean language interpreter must be available at these border checkpoints to help passengers to complete the form and assist public officers in sorting out completed declaration forms.
