The latest forms of application for issuance, re-issuance and modification of the pharmaceutical practice certificate are specified as follows:
- Application form for issuance of the pharmaceutical practice certificate: Form No. 02 attached to the Appendix I;
- Application form for re-issuance of the pharmaceutical practice certificate: Form No. 04 attached to the Appendix I;
- Application form for modification of the pharmaceutical practice certificate: Form No. 05 attached to the Appendix I.
Applicants for issuance, reissuance and modification of the pharmaceutical practice certificate should see details of required documents given in Article 3, 4 and 5 of this Decree, particularly enclosing the followings:
+ 2 portrait photos, 4x6 cm size, taken in front of the plain white background in the last 6 months;
+ Original or authenticated duplicate copy of the health certificate issued by the competent healthcare service providers;
+ Documents issued by foreign authorities which must be legalized by the Consular Authority, translated into the Vietnamese language and legally notarized.
More details are given in the Appendix to the Decree No. 54/2017/ND-CP that is going to enter into force from July 1, 2017.
