Re. providing instructions about procedures
for verification of Covid-19 vaccination information and Covid-19 vaccination

- People’s Committees of provinces and centrally-affiliated cities;
- MOH-controlled units;
- Health agencies of Ministries and regulatory authorities.
(hereinafter collectively referred to as authorities)
With a view to carrying out the Prime Minister’s
instructions, the Ministry of Health is running the COVID-19 vaccination
program. Up to now, 62 million doses of vaccine have been administered to 63%
of people over 18 years of age, many of whom receive missing, omitted or
inaccurate vaccination information after being vaccinated. This situation leads
to difficulties in giving vaccinated citizens confirmation of their vaccination
status.
On October 16, 2021, the Ministry of Health, the
Ministry of Public Security and the Ministry of Information and Communications
convened to reach their agreement on sharing and verification of COVID-19
vaccination information on the national population database.
With the aim of ensuring uniform implementation and
completeness of vaccination information, the Ministry of Health requests
authorities to instruct their relevant units to perform tasks or activities
involved in the Procedures for verification of Covid-19 vaccination information
and Covid-19 vaccination that are issued as an annex to this Official Dispatch,
as follows:
1. The Ministry of Health
would like the Ministry of Public Security to instruct its directly controlled
units and local police to cooperate with local authorities and health
authorities in carrying out activities according to the procedures; making
reports on results of verification of citizen information on a daily basis; and
consolidating them into a comprehensive report for submission to the Ministry
of Health.
2. The Ministry of Health
would like the Ministry of Information and Communications to instruct the
Departments of Information and Communications and relevant units to cooperate
and carry out activities according to the procedures, and communicate provision
of accurate personal information to the public on mass media when they receive
COVID-19 vaccines; take charge of developing fully-equipped software to meet
requirements of tasks or activities involved in the procedures; take charge of
assisting in issuing public digital certificates to relevant units to digitally
sign vaccination data on the COVID-19 Immunization Management Platform.
3. The Ministry of Health
requests People’s Committees of provinces and centrally-affiliated cities to
instruct the Departments of Health, Police of provinces and cities and local
authorities to collect, update and verify COVID-19 vaccination information
according to the procedures and report on vaccination results as legally
required.

PROCEDURES FOR VERIFICATION
OF COVID-19 VACCINATION INFORMATION AND COVID-19 VACCINATION

I. At local jurisdictions

Presidents of People's Committees of communes,
wards and townships (also known as communal-level People’s Committees) instruct
commune-level health stations to cooperate with commune-level police forces (in
combination with the primary survey of people of legal age to obtain ID cards),
residential quarters, community-based COVID protection teams, etc. in making
lists (by receiving registration information for vaccination directly or via
data available on the Covid-19 vaccination management platform that are
provided by people themselves or their host institutions) of those wishing to
get the first dose of vaccine, expecting the second dose of vaccine, and
sending them to commune-level police stations to check and verify registration
information.
Step 2: Commune-level police check registration
information against information available in the population database to request
any revised or further information, then confirm such information and send it
back to commune and ward-level health stations for their administrative
purposes. Verification of information about non-Vietnamese citizens is not
required.
If information about any person on the list is
missing or incorrect in comparison with information available in the population
database, the commune-level Police must check whether any additional
information is needed and re-send the revised lists to commune-level health
stations.
Time limit for resending the revised list shall not
be longer than 02 days from the day on which the health station makes the list.
Step 3: Based on data confirmed by the Police,
commune-level health stations draw up and execute the plans to vaccinate people
under the guidance of the Ministry of Health (prioritizing formulation of the
general plan, and performing the combined tasks of issuance of ID cards and
Covid-19 vaccination) for people’s convenience).
Persons who come to receive vaccines must bring
their ID cards or provide their personal identification numbers with QR codes
issued by the Police and certificates of the preceding dose of COVID-19 vaccine
(if any).
Vaccination sites check information and provide any
update or supplement to existing information, administer vaccines to people
with correct information, enter information about new injections on the
COVID-19 Immunization Management Platform.
Step 4: After data about vaccinated persons are
completely input, Presidents of the commune-level People's Committees sign
digital certificates for data on vaccination results on the COVID-19
Immunization Management Platform. While awaiting digital certificates,
Presidents of commune-level People's Committees take charge of obtaining
written confirmation of accuracy of data on vaccination results from involved
entities.
II. At COVID-19
vaccination units hosting COVID-19 vaccination sessions
