Accordingly, the order of handling the procedure for deduction of payment in the renewal of health insurance cards for households on the Public Service Portal includes 5 steps.
In which, in Step 1, the declarant chooses to declare on the National Service Portal or on the Social Security Service Portal and make a full declaration of information fields, including:
- Information of the Head of Household: Full name, date of birth; sex; personal identification number;
- List of members with the same permanent residence registration or temporary residence registration requesting extension of the health insurance card according to the family: health insurance card number, full name, date of birth, gender, relationship with the head of household, number month of request for extension.
