Kính gửi:

- Bệnh viện trực thuộc Bộ Y tế; - Sở Y tế các tỉnh, thành phố trực thuộc Trung ương; - Y tế các Bộ, ngành.

Trong bối cảnh tình hình dịch bệnh COVID-19 hiện nay, số lượng người mắc COVID-19 đã gia tăng ở khắp các tỉnh, thành phố, trong đó một số người dân xuất hiện các dấu hiệu, triệu chứng kéo dài, ảnh hưởng đến sức khỏe hoặc khả năng lao động và có nhu cầu đi kiểm tra sức khỏe. Để thực hiện hiệu quả công tác khám, chữa bệnh cho người dân sau khi mắc COVID-19 trong giai đoạn hiện nay, Bộ Y tế chỉ đạo:

1. Giám đốc Sở Y tế các tỉnh, thành phố trực thuộc Trung ương, Thủ trưởng các đơn vị trực thuộc Bộ Y tế và Y tế các Bộ, ngành nghiêm túc tổ chức triển khai, thực hiện một số nội dung sau:

a) Các cơ sở khám bệnh, chữa bệnh thực hiện:

- Khám bệnh, chữa bệnh cho người bệnh (người lớn và trẻ em) khi mắc và sau mắc COVID-19 theo đúng các hướng dẫn chuyên môn do Bộ Y tế đã ban hành, như Hướng dẫn chẩn đoán và điều trị COVID-19, trong đó có hướng dẫn phục hồi chức năng cho người bệnh COVID-19 sau ra viện, chăm sóc sức khỏe tâm thần; Hướng dẫn Phục hồi chức năng bệnh Viêm đường hô hấp cấp tính do SARS-CoV-2 (COVID-19)...

- Khám bệnh, chữa bệnh cho người có dấu hiệu, triệu chứng sau khi mắc COVID-19 (sau đây gọi tắt là hậu COVID-19) nói riêng và khám bệnh, chữa bệnh nói chung theo phạm vi hoạt động chuyên môn của cơ sở, theo các lĩnh vực chuyên khoa khi người dân có các dấu hiệu, triệu chứng thuộc các chuyên khoa, tránh tình trạng lạm dụng chỉ định hoặc chỉ định không phù hợp, không cần thiết.

- Thực hiện công tác truyền thông, quảng cáo công tác khám bệnh, chữa bệnh, khám sau COVID-19 phải tuân thủ theo quy định tại Thông tư số 09/2015/TT-BYT ngày 25/5/2015 của Bộ Y tế quy định về xác nhận nội dung quảng cáo đối với sản phẩm, hàng hóa, dịch vụ đặc biệt thuộc lĩnh vực quản lý của Bộ Y tế.

- Tăng cường kiểm tra, giám sát các hoạt động khám bệnh, chữa bệnh tại các khoa, phòng chuyên môn.

b) Chỉ đạo các cơ quan truyền thông, cơ sở khám bệnh, chữa bệnh tăng cường truyền thông, nâng cao nhận thức để người dân hiểu đúng và đầy đủ về các dấu hiệu, triệu chứng hậu COVID-19, thời điểm người dân cần đi khám bệnh, chữa bệnh và thực hiện khám bệnh, chữa bệnh phù hợp, tránh để người dân lo lắng, hoang mang quá mức, tránh lạm dụng chỉ định ở các cơ sở khám bệnh, chữa bệnh. Một số nội dung chính cần truyền thông như sau:

- Sau mắc COVID-19 một số người chậm hồi phục sức khỏe cần được theo dõi sức khỏe.

- Hậu COVID-19 xảy ra ở người có tiền sử nhiễm SARS-CoV-2 có các triệu chứng xuất hiện trong hoặc sau khi mắc COVID-19, kéo dài 12 tuần và không thể lý giải bằng các chẩn đoán khác (theo Tổ chức Y tế thế giới và Viện NICE - Vương Quốc Anh).

- Các triệu chứng hậu COVID rất đa dạng: có đến 203 triệu chứng khác nhau, có thể xuất hiện sau khi COVID-19 đã hồi phục, hoặc tồn tại dai dẳng từ đầu, hoặc tái phát theo thời gian. Triệu chứng phổ biến nhất bao gồm mệt mỏi, khó thở, đau cơ xương khớp, giảm hoặc mất vị giác hoặc khứu giác, suy giảm nhận thức, rối loạn giấc ngủ, ho, đau ngực…

- Khi các dấu hiệu, triệu chứng của hậu COVID-19 làm sức khỏe con người bị suy giảm kéo dài, tác động đến khả năng quay trở lại làm việc hoặc tham gia cuộc sống xã hội, người dân cần đi khám sức khỏe.

- Các cơ sở khám bệnh, chữa bệnh đa khoa hoặc chuyên khoa thực hiện khám bệnh, chữa bệnh hậu COVID-19 theo phạm vi hoạt động chuyên môn được cấp phép.

- Người dân không sử dụng các loại thuốc không rõ nguồn gốc, thuốc không được cấp phép lưu hành sản phẩm hay các bài thuốc truyền miệng hoặc theo các nguồn tin không chính thống để chữa bệnh.

- Một số nội dung truyền thông khác phải dựa trên các khuyến cáo, hướng dẫn, bằng chứng khoa học tin cậy trong nước và quốc tế.

2. Giao Cục Quản lý Khám, chữa bệnh, Thanh tra Bộ, Sở Y tế các tỉnh, thành phố, Y tế các Bộ, ngành tăng cường kiểm tra, giám sát các cơ sở khám bệnh, chữa bệnh trực thuộc và cơ sở y tế tư nhân trên địa bàn về hoạt động chuyên môn, thông tin, quảng cáo khám chữa bệnh để kịp thời giải quyết các khó khăn, vướng mắc, chấn chỉnh, xử lý nếu có vi phạm.

3. Giao Cục Quản lý Khám, chữa bệnh phối hợp với các Vụ, Cục liên quan, các nhà chuyên môn tiếp tục nghiên cứu, cập nhật, đề xuất ban hành các hướng dẫn chuyên môn và triển khai tập huấn để nâng cao chất lượng khám, chữa bệnh cho người bệnh.

Trên đây là ý kiến của Bộ Y tế, đề nghị các đơn vị triển khai thực hiện./.

To:

- Hospitals affiliated to Ministry of Health;
- Health Departments of provinces and central-affiliated cities;
- Health authorities of ministries and central governments.

Amidst COVID-19 and the increase of COVID-19 cases in all provinces and cities, some people exhibit prolonged symptoms and/or signs that affect their health and/or working capacity and thus need to have a health check-up. In order to effectively implement medical examination and treatment for people who have recovered from COVID-19, the Ministry of Health hereby orders:

1. Directors of Health Departments of provinces and central-affiliated cities, heads of entities affiliated to the Ministry of Health, and health authorities of ministries and central governments to organize implementation of the followings:

a) Direct medical examination and treatment establishments to:

- Provide medical examination and treatment for adults and children who have been infected and recovered from COVID-19 in accordance with professional guidelines promulgated by the Ministry of Health such as the Guidelines on diagnosis and treatment of COVID-19, which guides rehabilitation for COVID-19 patients discharged from hospitals and mental health care; Guidelines on rehabilitation following COVID-19 infection, etc.

- Provide medical examination and treatment for people experiencing symptoms or signs following COVID-19 infection (hereinafter referred to as “post COVID-19”) and provide general medical examination and treatment within professional departments of the establishments when patients exhibit symptoms appropriate to the professional departments; prevent excessive prescription.

- Communication and advertisement affairs in medical examination and treatment and post COVID-19 examination must comply with Circular No. 09/2015/TT-BYT dated May 25, 2015 of Ministry of Health on confirmation of advertisement contents for special products, goods, and services under management of Ministry of Health.

b) Direct communication authorities and medical examination and treatment establishments to increase communication and increase awareness of the general public regarding post COVID-19 symptoms, signs, the time in which people should get medical examination and treatment in order to prevent confusion among the general public, avoid excessive prescription in medical examination and treatment establishments. Primary details to be communicated include:

- Following COVID-19 infection, some people having slow recovery need to have their health monitored.

- Post COVID-19 symptoms occur in people previously having SARS-CoV-2 infection. These post COVID-19 symptoms develop either during or following a COVID-19 infection, continue for 12 weeks, and cannot be explained by an alternative diagnosis (according to World Health Organization and National Institute for Health and Care Excellence - United Kingdom).

- Post COVID-19 symptoms are vast with up to 203 different symptoms which can occur after a patient has recovered from COVID-19, or linger from the beginning of a COVID-19 infection, or reappear. The most common symptoms include tiredness, dyspnea, musculoskeletal pain, reduced or lost sense of taste or smell, cognitive decline, sleep disorder, coughing, chest paint, etc.

- It is recommended that a person gets a health check-up if his/her health is deteriorated over a long period of time, working capacity or social life is affected as a result of post COVID-19 symptoms.

- General and field-specific medical examination and treatment establishments shall provide post COVID-19 medical examination and treatment within the licensed fields of operation.

- Do not use medicine with unclear origin, or medicine which is not approved for market authorization, or folk medicine or unofficial information sources for treatment purposes.

- Rely on national and international recommendations, guidance, and reliable scientific proof to implement other communication contents.

2. Assign Department of Medical Service Administration, Inspectorate of Ministry of Health, Health Departments of provinces and cities, health authorities of ministries and central governments shall increase inspection and supervision of affiliated medical examination and treatment establishments and private medical establishments in terms of professional operation, information, and medical examination and treatment advertisement in order to promptly deal with difficulties, rectify, and take actions.
