Kính gửi:

Bộ Y tế nhận được văn bản đề xuất của nhiều Sở Y tế tỉnh, thành phố trực thuộc Trung ương đề nghị cho ý kiến về việc sử dụng kết quả xét nghiệm test nhanh kháng nguyên để xác định người nhiễm SARS-CoV-2 và xác định tình trạng khỏi bệnh và cho ra viện, sau khi nghiên cứu, Bộ Y tế có ý kiến như sau:

1. Hiện nay tình hình dịch bệnh COVID-19 trên cả nước diễn biến phức tạp, số mắc có xu hướng ngày một tăng, để thích ứng tình hình mới và bảo đảm an toàn, linh hoạt phù hợp với điều kiện và nguồn lực của địa phương, đề nghị các Sở Y tế tỉnh, thành phố, các Bệnh viện trực thuộc Bộ Y tế, Y tế các Bộ, ngành căn cứ cấp độ dịch, khả năng đáp ứng và Hướng dẫn tạm thời về quản lý người nhiễm COVID-19 tại nhà ban hành kèm theo Quyết định số 4038/QĐ-BYT ngày 21/8/2021 của Bộ trưởng Bộ Y tế, Hướng dẫn chẩn đoán, điều trị COVID-19 ban hành kèm theo Quyết định số 4689/QĐ-BYT ngày 06/10/2021 để xác định ca bệnh theo hướng dẫn:

a) Là trường hợp bệnh nghi ngờ (theo định nghĩa tại mục 2.1 của Quyết định số 4689/QĐ-Bộ Y tế, ngày 06/10/2021 của Bộ Y tế) hoặc bất cứ người nào có xét nghiệm dương tính với vi rút SARS-CoV-2 bằng kỹ thuật Real - time RT-PCR.

b) Là trường hợp bệnh nghi ngờ (theo định nghĩa tại mục 2.1 của Quyết định số 4689/QĐ-BYT ngày 06/10/2021) có kết quả xét nghiệm kháng nguyên dương tính với vi rút SARS-CoV-2 (test nhanh do Bộ Y tế cấp phép và do NVYT thực hiện hoặc người nghi nhiễm thực hiện dưới sự giám sát của NVYT bằng ít nhất một trong các hình thức trực tiếp hoặc gián tiếp qua các phương tiện từ xa).

c) Là những người không có triệu chứng, có yếu tố dịch tễ hoặc tiếp xúc gần với trường hợp bệnh nghi ngờ hoặc xác định COVID-19 trong khoảng 14 ngày và có 02 kết quả xét nghiệm kháng nguyên dương tính với vi rút SARS-CoV-2 (xét nghiệm lần 2 trong vòng 8 giờ kể từ khi có kết quả xét nghiệm lần 1, test nhanh do Bộ Y tế cấp phép và do NVYT thực hiện hoặc người nghi nhiễm thực hiện dưới sự giám sát của NVYT bằng ít nhất một trong các hình thức trực tiếp hoặc gián tiếp qua các phương tiện từ xa). Trong trường hợp chỉ có 01 kết quả xét nghiệm kháng nguyên dương tính với vi rút SARS-CoV-2 thì cần phải có xét nghiệm Real-time RT-PCR để khẳng định.

2. Về đề xuất sử dụng kết quả xét nghiệm kháng nguyên để xác định tình trạng khỏi bệnh và cho ra viện:

a) Đối với người bệnh COVID-19 không triệu chứng hoặc triệu chứng nhẹ đủ điều kiện cách ly, điều trị tại nhà theo quy định sẽ được dỡ bỏ cách ly, điều trị tại nhà khi:

- Thời gian cách ly, điều trị đủ 10 ngày

- Kết quả xét nghiệm kháng nguyên âm tính với vi rút SARS-CoV-2 do NVYT thực hiện hoặc người bệnh tự thực hiện dưới sự giám sát của NVYT bằng ít nhất một trong các hình thức trực tiếp hoặc gián tiếp qua các phương tiện từ xa (test nhanh do Bộ Y tế cấp phép)

- Trạm Y tế nơi quản lý người bệnh chịu trách nhiệm xác nhận khỏi bệnh cho người bệnh.

b) Đối với người bệnh COVID-19 nằm điều trị các các cơ sở thu dung, điều trị

* Người bệnh COVID-19 đơn thuần

- Các triệu chứng lâm sàng hết trước ngày ra viện từ 3 ngày trở lên

- Có kết quả xét nghiệm bằng phương pháp Real-time RT-PCR âm tính với SARS-CoV-2 hoặc nồng độ vi rút thấp (Ct ≥ 30) hoặc xét nghiệm kháng nguyên âm tính với vi rút SARS-CoV-2 vào trước ngày ra viện (test nhanh do Bộ Y tế cấp phép).

- Người bệnh sau khi ra viện cần ở tại nhà và tự theo dõi trong 7 ngày. Đo thân nhiệt 2 lần/ngày. Nếu thân nhiệt cao hơn 38°C ở hai lần đo liên tiếp hoặc có bất kỳ dấu hiệu lâm sàng bất thường nào thì cần báo cho y tế cơ sở để thăm khám và xử trí kịp thời.

- Tuân thủ thông điệp 5K

* Người bệnh COVID-19 có bệnh nền hoặc bệnh kèm theo

- Các triệu chứng lâm sàng của bệnh COVID-19 hết trước ngày ra viện từ 3 ngày trở lên

- Có kết quả xét nghiệm bằng phương pháp Real-time RT-PCR âm tính với SARS-CoV-2 hoặc nồng độ vi rút thấp (Ct ≥ 30) hoặc xét nghiệm kháng nguyên âm tính với vi rút SARS-CoV-2 vào trước ngày ra viện (test nhanh do Bộ Y tế cấp phép).

- Người bệnh được chuyển sang khoa điều trị bệnh kèm theo hoặc khoa điều trị bệnh nền (nếu cần) tại buồng riêng của khoa đó để tiếp tục điều trị và được sàng lọc, theo dõi theo quy định đối với người bệnh nội trú. Đo thân nhiệt 2 lần/ngày.

- Tuân thủ thông điệp 5K

Trên đây là ý kiến của Bộ Y tế, đề nghị các đơn vị triển khai thực hiện, theo dõi, đánh giá và báo cáo kết quả về Bộ Y tế để kịp thời điều chỉnh cho phù hợp./.

- Provincial Departments of Health;
- Health agencies affiliated to regulatory Ministries;

In response to requests of Provincial Departments of Health for guidelines on use of antigen-detecting rapid diagnostic test (Ag-RDT) for confirming people infected with SARS-CoV-2 and determining patients recovered from Covid-19 and eligible for discharge from hospitals, the Ministry of Health hereby gives some guidelines hereunder:

1. Now complicated developments of the COVID-19 pandemic are taking place throughout the country. New COVID-19 cases have been rising. In order to facilitate the safe and flexible adaption to the new context of pandemic as well as actual conditions and resources of each province/city, Provincial Departments of Health, hospitals affiliated to the Ministry of Health and health agencies affiliated to regulatory Ministries are requested to, based on levels of infection and coping and adaptive capacity, and pursuant to Provisional guidelines for management of people infected with COVID-19 at home enclosed with the Minister of Health’s Decision No. 4038/QD-BYT dated August 21, 2021 and Guidelines for diagnosis and treatment of people infected with COVID-19 enclosed with the Decision No. 4689/QD-BYT dated October 06, 2021, determine the following cases:

a) Suspected cases (as defined in Section 2.1 of the Ministry of Health’s Decision No. 4689/QD-BYT dated October 06, 2021) or any persons who have tested positive for SARS-CoV-2 with Real-time RT-PCR test.

b) Persons meeting suspected case criteria (as defined in Section 2.1 of the Ministry of Health’s Decision No. 4689/QD-BYT dated October 06, 2021) and having positive SARS-CoV-2 antigen-detecting rapid diagnostic test (Ag-RDT) (the rapid test kits must be licensed by the Ministry of Health and the test must be done by a health worker or a person suspected of COVID-19 infection under either direct or indirect supervision of the health officer).

c) Asymptomatic persons meeting epidemiological criteria or having close contact with a suspected or confirmed case within 14 days and having positive SARS-CoV-2 Ag-RDT twice in a row (the 2^nd test is done within 8 hours from time of obtaining the 1^st test result; the rapid test kits must be licensed by the Ministry of Health and the test is done by a health worker or a person suspected of COVID-19 infection under either direct or indirect supervision of the health worker). If only a positive SARS-CoV-2 Ag-RDT result is given, the Real-time RT-PCR is required for confirmation.

2. Regarding the use of Ag-RDT for determining patients recovered from Covid-19 and eligible for discharge from hospitals:

- He/she has completed 10-day home isolation and treatment.

- He/she has a negative Ag-RDT which is performed by a health officer or self-performed under either direct or indirect supervision of a health officer (the rapid test kit must be licensed by the Ministry of Health).

- The medical station in charge of managing the patient shall assume responsibility to certify recovery from COVID-19 to the patient.

b) Regarding COVID-19 patients receiving treatment at COVID-19 treatment facilities:

*COVID-19 patients without comorbidities

- The patient has no clinical symptoms at least 3 days before discharge

- The patient has tested negative for SARS-CoV-2 with Real-time RT-PCR test or has CT value ≥ 30 or has a negative SARS-CoV-2 Ag-RDT result given before discharge (the rapid test kit must be licensed by the Ministry of Health).

- After discharge, the patient is required to isolate himself/herself at home and self-monitor his/her health for further 7 days. The patient’s body temperature should be checked twice daily. If the temperature is higher than 38°C in two consecutive measures or there are any abnormal clinical symptoms, the patient should notify the health facility for immediate examination and actions.

- The patient must comply with 5K Message

- The patient has no clinical symptoms for COVID-19 at least 3 days before discharge

- The patient has tested negative for SARS-CoV-2 with Real-time RT-PCR test or has CT value ≥ 30 or has a negative SARS-CoV-2 Ag-RDT result given before discharge (the rapid test kit must be licensed by the Ministry of Health).

- The patient is transferred to receive treatment of comorbidity (if any) and is screened and monitored as an inpatient at a separate room of the relevant ward. The patient’s body temperature should be checked twice daily.

- The patient must comply with 5K Message

Relevant agencies and units are requested to comply with all guidelines herein and report implementation results to the Ministry of Health for consideration./.
